accomplishments. The Quality (Q), Efficiency (E) and Timeliness (T) shall be used
                       in giving each accomplishment a numerical rating.

                   3.  The supervisor shall add all the scores under Q, E and T for each work activity of the
                       ratee and get the average point. To get the weighted average for each performance
                       activity, the average scores under Q, E and T for each performance indicator shall
                       be multiplied with the corresponding weight of each activity.

                   4.  The  supervisor  shall  multiply  the  average  point  by  the  assigned  weight  of  the
                       performance indicator and get all the sum of weighted average point. To get the final
                       rating per success indicator, the weighted average shall be added.

                   5.  To compute the final performance rating, the Overall Weighted Average Scores per
                       success indicator shall be added.
