of L4 and at the cranial end of L5 (c). An osteodestructive mass is seen within the body, left pedicle, lamina, and vertebral canal of L4
(a,b,d: arrows) and has extended caudally into the body of L5 (c: arrow). CT myelography confirms left‐sided spinal cord compression
(a,b,e: arrowhead). There are also incidental findings of ventral spondylosis (c: arrowhead) and an L3–4 vacuum sign (d: arrowhead).
Postmortem examination revealed osteosarcoma. The imaging features of this neoplasm are somewhat unusual given the purely
destructive appearance that suggests an osteoclastic or primitive cell lineage.
