3. Place fingertips of your middle three fingers (Do not use the thumb) along the groove
                       at the base of the patient's wrist.
                       4. Press against the radial artery to obliterate pulse then gradually release pressure until
                       pulsation is felt.
                       5. Assess pulse for regularity & strength.
                       6. If the pulse is regular, count pulse for 15 seconds and multiply by four. If slue is
                       irregular, count for a full minute.
                       7. Document your findings.

                       Apical pulse
                       1. Position patient in the supine or sitting position with sternum and left chest exposed.
                       2. Warm diaphragm of the stethoscope by holding in the palm of your hand for 5 to 10
                       seconds.
                       3. Insert the earpieces of the stethoscope into your ears & place the diaphragm over the
                       apex of the patient's heart (fifth intercostal space, near the midclavicular line).
                       4. Assess the heartbeat for regularity.
                       5. If the rhythm is regular, count the heartbeat for 30 seconds& multiply by two for
                       irregular heartbeat count for 1 full minute.
                       6. Replace the patient's gown and assists in returning to a comfortable position.
                       7. Chart your finding on vital signs record.
                       15.2 Doing Neurological examination
                       Neurological examination may be included with the physical examination

                       To investigate the efficiency of the nervous system, i.e sensory and motor reflexes
                       Equipment
                   1.  Ophthalmoscope
                   2.  Auriscope and tuning fork
                   3.  Small bottles with distinctive smelling liquid
                   4.  Salt, sugar or vinegar with a dropper or teaspoon
                   5.  Spatula and torch
                   6.  Pins
                   7.  Wool in galipot
                   8.  Hot and cold water in a separate test tube
                   9.  Tape measure
                   10. Skin pencil, creaser
                   11. Coin, key
                   12. Tendon hammer(percussion hammer)
                   13. B/P apparatus
                   14. Receiver and a glass of water for the patient to rinse his mouth during testing of test
                       sensation
