Equipment
                   1.  In and out form at the bedside
                   2.  In and out graphic record in the chart
                   3.  Glass or cup
                   4.  Bedpan,  urinal, or bedside commode
                   5.  Graduated container for output
                   6.  Non-sterile gloves

                       Procedures
                   1.  Wash hands.
                   2.  Explain the purpose of keeping intake and output records to the client. Explain that:
                   3.  All fluids taken orally must be recorded.
                   4.  Forms for recording must be used.
                   5.  The client must void into a bedpan or urinal, not into the toilet.
                   6.  Toilet tissue should be disposed of in a plastic-lined container, not in a bedpan.
