This scheme is a bona fide contribution from PPZ-MAIWP SCHEME INFORMATION
to the Benefactor of the PPZ-MAIWP Zakat Payer in the
event of death and subject to the set terms and conditions. What Should an Applicant is to complete the Kafalah Muzakki
Applicant Do? Scheme Form and submit the form at any of the
PPZ-MAIWP counters in the Federal Territory. The
form can be obtained at the PPZ-MAIWP counters or
downloaded at the official portal,
www.zakat.com.my.
The Documentations that MUST Accompany the
Application Form
1. Death Certificate (original and copy)
2. Copy of the Deceased Identification Card (if any)
3. Applicant’s Identification Card (original and copy)
4. Documentation of the relationship between the
Deceased and the Applicant (original and copy of
certificate) *
i. Marriage Certificate
ii. Birth Certificate (The Deceased or Applicant)
5. A copy of a bank passbook with the account
number or a copy of a bank account statement of
the Applicant.
6. A Third-Party Applicant must also submit a
Letter of Authorization from the Next of Kin or a
Statutory Declaration.
*The Document depends on the relationship between the Deceased and the Applicant
