The Leave Process (cont'd)
► Requesting Leave
► Employee must notify Manager
► Employee must notify The Hartford* (1-888-301-5613 or
www.TheHartfordatWork.com) 30 days prior to expected last day of work (or as
soon as possible) – 1199 employees must also contact the union to apply for
disability benefit
► Manager receives leave determination notification from The Hartford
► Extending Leave
► The Hartford works with employee to determine return to work date or extension
► If extension requested, obtains necessary information from both employee and
treating physician to make determination
► Manager receives extension determination notification

Family and Medical Leave Act
Provides up to 12 weeks of job protected (continuous or
intermittent), unpaid leave of absence, within a 12-month
period for specified family and medical reasons (26
weeks for military caregivers).
Eligibility:
► Worked for VNSNY for at least 12 months;
► Worked at least 1,250 hours over the previous 12 months;
► Not used up his/her FMLA entitlement in previous 12 month
period.
