Analyzing and Determining Diagnostic Priorities
Comprehensively analyzing patient symptoms and determining diagnostic
priorities for effective care planning
Read each scenario carefully and study the excerpt dialogue between the nurse and the patient. Identify
and categorize the following information:
1. List of Symptoms
2. Main Priority
3. Goals and Criteria
4. Types of Interventions
Write your answers in the provided table based on the details from the dialogue.
Scenario 1: Post-Surgery Complications
Case: Mr. Ahmed, a 50-year-old patient recovering from abdominal surgery, reports moderate pain
around the surgical site, difficulty walking due to fatigue, and signs of redness and swelling near
the incision.
Nurse : “Assalamu alaikum, Mr. Ahmed. How are you feeling today?”
Mr. Ahmed : “Wa alaikum assalam. I’ve been feeling pain around my incision, and it’s hard to move.
I’ve also noticed some swelling and redness near the surgical area.”
Nurse : “Thank you for sharing that. Our priority is to address the risk of infection first since
swelling and redness could indicate complications. Next, we’ll work on your mobility to
help with recovery. Finally, we’ll manage your pain with appropriate medication.”
Nurse : “Our goals are: (1) to reduce redness and swelling within 48 hours, (2) to help you walk
short distances by the end of the week, and (3) to bring your pain level to below 4 on a
scale of 0 to 10.”
Nurse : “To achieve these, I’ll start by cleaning and dressing your wound to prevent infection and
administer antibiotics. I’ll also guide you in gentle exercises to improve circulation and
mobility, and I’ll give you a pain reliever every six hours as needed.”
Scenario 2: Respiratory Issues
Case: Mrs. Sarah, a 60-year-old patient with chronic bronchitis, complains of shortness of breath
during light activities, a persistent cough, and mild chest pain.
Nurse : “Assalamu alaikum, Mrs. Sarah. How have you been managing your symptoms?”
Mrs. Sarah : “Wa alaikum assalam. It’s been tough. I feel breathless even when walking short
distances, and my cough is constant. Sometimes, I get mild chest pain too.”
Nurse : “I’m sorry to hear that. The first priority is to improve your breathing by ensuring your
airways are clear and your oxygen levels are stable. Next, we’ll focus on addressing your
persistent cough. Lastly, we’ll monitor and manage the chest pain to ensure it doesn’t
worsen.”
Nurse : “Our goals are: (1) to maintain oxygen saturation above 92% at rest, (2) to reduce
coughing episodes by 50% within 3 days, and (3) to minimize chest pain during
activities.”
