The populations we serve are diverse in many ways. They are majority male
(70%) and majority people of color (67%). They include veterans, seniors,
those with serious mental illnesses and those who have been formerly

incarcerated. Most have significant health concerns: 57% mental illness, 73%
disabling conditions and 88% chronic conditions. It is not unusual for many to

experience intersectionality, with many belonging to more than one social
identity or marginalized group.
We recognize the many inequities faced by the people we serve as they

interface with the health care system. We will continue our current efforts for

cutting edge solutions designed to integrate housing and supportive services
and expand our footprint — geographically and through our services — for

greater impact and results.
