DIABETES MELLITUS
Management Plan:
People with diabetes should receive medical care from a
collaborative, integrated team with expertise in diabetes. This team may
include physicians, nurse practitioners, physician’s assistants, nurses,
dietitians, pharmacists, and mental health professionals. Individuals
with diabetes must also assume an active role in their care.
The objectives of diabetes management are:
To reduce symptoms of diabetes
To achieve optimum glycemic control.
To prevent development or progression of diabetic complications.
To enable affected individuals to maintain their quality of life
(QOL) and life expectancy.
Glycemic control:
Optimal glycemic control is fundamental to the management of
diabetes.
Glycemic targets should be individualized based on the
individual’s age, duration of diabetes, risk of severe hypoglycemia,
presence or absence of cardiovascular disease and life expectancy.
It is important that hyperglycaemia, hypertension and
dyslipidaemia are all controlled to reduce the risk of development
or progression of complications.
The ABC’s of Diabetes management:
The “ABCs of Diabetes” are defined as :
Hemoglobin A1c < 7.0%,
Blood pressure < 130/80 mm Hg, and
Low- density lipoprotein cholesterol (LDL-C) <100 mg per dl.
