Part A: The New Student
            1.  The Admission Office:  Once an enrollment contract has been signed and returned to the school, the
               Admission Office (if it is aware of the new student's life-threatening food allergy) should immediately
               notify the school nurse and the student's division head. Depending on the time of year, the school
               nurse (or division head during the summer) will make sure that the process, beginning with #2 below,
               begins.
            2.  The Student's Parent(s)/ Designated Medical Physician: Before the end of August, parents of a new
               student with life threatening food allergies must communicate in writing with the school about the
               details of their student's allergic condition. Written documentation from the student's designated
               medical physician (who supervises the allergic condition) about the nature of the life-threatening food
               allergy also must be provided. The student's designated physician must provide written information
               about "safe and unsafe" foods to the school. Parents must also give the school written permission to
               discuss their student's food allergy with appropriate Renbrook employees and SAGE Dining Services
               staff.
            3.  The School Nurse: The school nurse is responsible for: ensuring #2 (listed above) occurs; for
               creating/maintaining a file on the student; and ensuring that the appropriate Renbrook employees and
               SAGE Dining Services staff are aware of the student's health needs. A coordination of efforts with the
               student's classroom faculty, SAGE Dining Services staff, the Chief Financial Officer, and the student's
               parents is essential. The nurse will also share the student's food allergy information with Renbrook
               School's emergency team. Although the nurse is the primary administrator of the EpiPen, all emergency
               team individuals are trained in administering the EpiPen.
            4.  The Division Head: During faculty meetings before school opens, the division head will inform faculty
               about the specifics of the student's food allergies and what to do in case of an emergency should food
               allergy symptoms appear, and will implement procedures designed to maintain the student's safety. In
               addition, during the first half of September, the division head will communicate the food allergy issues
               with the parent representatives of the student's grade. Finally, they will ask for periodic updates from
               the classroom faculty (or Upper School advisor).

            5.  The Classroom Faculty: During the year, the classroom faculty (or Upper School advisor) is responsible
               for making sure there is good communication about the student's food allergy issues among the
               student's parents, the school nurse, the director of SAGE Dining Services, and appropriate Renbrook
               employees. The faculty will provide daily assistance to the student (as appropriate to the student's age)
               about safe food choices, and will enlist the support of classmates in order to maintain a safe school
               environment for that student. The faculty will also make sure that the student's food allergy concerns
               are successfully managed during special events and/or field trips.

            6.  The Director of SAGE Dining Services: Is responsible for creating a safe food menu and clean dining
               room facility according to the student's specific life-threatening allergy(ies). He must inform his staff of
               the student's food allergy, and he may designate one or more of his staff to be specifically responsible
               for establishing and maintaining a safe food menu. A coordination of efforts with the student's
               classroom faculty, the school nurse, the Chief Financial Officer, and the student's parents is essential.

            7.  Chief Financial Officer: Is responsible for ensuring that there are regular and effective communications
               amongst those responsible and will request periodic updates from the school nurse, the classroom
               faculty (or Upper School advisor), and director of SAGE Dining Services.

            8.  All adults serving food or monitoring the lunchroom will be notified of the student's food allergy issues,
               and be made aware of what to do in case of an emergency should food allergy symptoms appear.
