        should notify your immediate supervisor or department manager as soon as possible. You may use PTO for the
        time.

        If operations are officially closed due to extreme emergency conditions, the time off from scheduled work will
        be paid. Employees in essential operations may be asked to work on a day when operations are officially closed.
        Employees who can work from home or another remote location will be required to do so or use PTO.



        Leaves of Absence

        Family & Medical Leave (FML)

        Diversified provides unpaid, job-protected leaves of absence to eligible employees under the federal and state
        Family  Medical  Leave  laws.  As  leave  laws  differ  from  state  to  state,  please  check  with  the  PeopleTeam  for
        information on eligibility and benefits in your state. Both Diversified and employees who wish to take leave have
        specific rights and obligations under the laws.

        How to Apply for Leave
        You may request FML by contacting MetLife and completing the application process. Contact information can
        be obtained from The PeopleTeam. Requests for FML must be submitted in writing at least 30 days prior to the
        beginning of the leave period, or as soon as practical. If you are unable to submit a written request for FML in
        advance because of unexpected qualifying circumstances (e.g., premature birth, medical emergency), you must
        provide oral or written notice to The PeopleTeam within 24 hours of the start of the leave or as soon as practical,
        and provide written notice within three (3) days after you return to work. The notice may be provided by another
        person on your behalf. Failure to provide the required notice for a foreseeable leave may delay the taking of
        leave until 30 days after the written request is submitted.

        Requests for FML will be reviewed by MetLife, a provider engaged by Diversified. MetLife will notify you if the
        leave request is approved or denied. Approved leaves will be counted against your annual leave period and will
        run concurrently with any qualifying disability or workers compensation leave. Failure to notify Diversified of the
        reason for any absence, whether partial or full day, which might qualify for FML could result in the absence being
        counted against you for attendance or other purposes (e.g., pay increases, promotional opportunities, etc.).


        Medical Certification
        You must provide a medical certification from your healthcare provider within 15 days of submitting a written
        request for leave or, if the need for leave is not foreseeable, within 15 days after the leave begins. Diversified will
        pay the cost of obtaining a required medical certification to the extent that it is not covered by insurance. Medical
        certification forms may be obtained from and returned to MetLife. Failure to provide the required certification
        may delay the taking of leave or prevent the request from qualifying as FML.

        If you have taken more than three days FML in a one-year period for sick child care, Diversified may require you
        to obtain at Diversified’s expense a medical certification from a qualified healthcare provider of the child’s need
        for you to provide care and thus miss time from work.
