Before restoring you to work after your own serious health condition, Diversified may require you to provide a
medical certification from your healthcare provider that you are able to resume work, and clearly explain any
work limitations or restrictions.
Continuation of Benefits
FML is unpaid. Employees taking FML will be required to first exhaust all accrued or available paid leave, which
will be counted toward the available FML for that year. In cases of a maternity leave, up to 40 hours can be
reserved for use upon return from leave.
While on approved FML, you will continue to receive the same level of health insurance benefits as if you had
continued to work, provided your contribution to health insurance premiums is timely paid by you during the
period of FML. If your required premium payment is more than 30 days late, you may be dropped from coverage
under Diversified’s health insurance plan.
Personal Leave
Diversified provides leaves of absence without pay to eligible regular full-time employees who wish to take time
off from work duties to fulfill personal obligations.
Vested PTO & Benefit Coverages
Eligible employees may request personal leave only after having completed three months of service. As soon as
eligible employees become aware of the need for a personal leave of absence, they should request a leave from
their supervisor and their PeopleTeam Business Partner.
Personal leave may be granted for a period of up to three months. Unpaid personal leave may only be requested
once all other appropriate leave balances have been exhausted.
While on approved Personal Leave, you will continue to receive the same level of health insurance benefits as if
you had continued to work, provided your contribution to health insurance premiums is timely paid by you
during the period of FML. If your required premium payment is more than 30 days late, you may be dropped
from coverage under Diversified’s health insurance plan. Benefit accruals will be suspended during the leave and
will resume upon return to active employment.
When a personal leave ends, every reasonable effort will be made to return the employee to the same position,
if it is available, or to a similar available position for which the employee is qualified. However, Diversified cannot
guarantee reinstatement in all cases. If an employee fails to report to work promptly at the expiration of the
approved leave period, Diversified will assume the employee has resigned.
Military Leave
A military leave of absence will be granted to employees who are absent from work because of service in the
United States uniformed services in accordance with the Uniformed Services Employment and Reemployment
