                         C.   All documentation related to the employee's or family member's medical condition will be
                              held in strict confidence and maintained in tbc employee's confidential file in the custody of
                              the Personnel Divisio11.

                         vm.  :EFFECT  N BENB17JTS
                         A.   An employee granted a leave under this policy will continue to be covered under the City of
                              Fannington's group health insurance plan and life insurance plan under the same conditions
                              as covenige would have been provided if1hey had been continuously employed during the
                              leave period.

                        B.    Employee contributions will be required either through payroll deduction or by direct
                              payment to the City ofFarn1ington.  '11te employee will be advised in writing at the
                              beginning of the leave period as to the amount and method of payment.  Employee
                              contdbution an10wils are subj cct to any change in rates that occurs while the employee is on
                              leave.

                        C.    Under cmTent City policy, the employee pays a portion of the health insurance premium.
                              While on paid leave, the City will continue to make payroll deductions to collect the
                              employee's share of the premium.  While on unpaid leave, the employee may either (A)
                              make payments of this portion in person or by mail  lo the Risk Management Division by the
                              tenth (10th) day of each month, or (B) at the beginning of the leave period, request the City
                              continue the pa)ments by signing a written statement that the employee will reimburse the
                              City for delinquent payments (on a payroll deduction schedule) upon ret11ming from the
                              leave.  1f payments as described in (A) are more than thirty (30) days late, coverage may be
                              suspended for the duration of the leave.   ·

                              The City will continue payment of premiums for basic life insurauce during the leave.  If the
                              employee carries optional suppl.emental life under the City plan, the same payment poJic.y
                              \Vil! apply as outlined in the paragraph above.
                        D.    lfthe employee fails to return from linpaid family/medical leave for reasons other than (1)
                              the continuation of a serious health condition of 1he employee or a covered family member,
                              or (2) circumstances beyond the employee's control (Cettification required within 30 days of
                              failure lo return for either reason), tlie City ofFanningion may seek reimbursement from the
                              employee for the portion of the premiums paid by the City of Farmington on behalf of that
                              employee (also k11own as the employer contribution) during the peliod ofkave.
                        E.    l\11 employee is not entitled to seniority or benefit accrual during periods ofR1LA leave
                              that exceed fourteen (14) calendar days, but will not lose. anything accrued ptior to leave.
