                           For upper diviftio11 and graduate courses, !he employee mnsl pro  ide ;i copy of th
                           indfridual.'s ctment plan of study, approved by the academic institution.  In order to
                           receive tuition reimbursement, at the conclusion of the class and within sixty days of the
                           completion of the class, the employee must submit verification of payments with a copy
                           of the transcript or grade report to the Persollllel Division.

                           Books and fees are not reimbursable expenses.

                           Section4.   Procedure fo:r Eligibility
                           The employee will complete the tuition reimbursement fonn prior to or immediately upon
                           enrolling in the class.  The form must be submitted to the employee's departme11t bead
                           who shall determine Ute job relatcclncss of the class.  Upon approval by the department
                           head, the tuition reimbursement form should be delivered to the Personnel Division
                           where it will remain through the completion of the class.

                           In order l.o  receive reimbursenient for a class, the employee must submit verification of
