RESOLUTION BY THE ADMINISTRATION, PERSONNEL, POLICY AND LEGAL
COMMITTEE AUTHORIZING ENTERING INTO A CONTRACT WITH DIVERSIFIED

BENEFITS SERVICES, INC. TO PROVIDE A HEALTH REIMBURSEMENT
ARRANGEMENT FOR VILLAGE EMPLOYEES

WHEREAS, the Village Employees lost health insurance coverage with Central States
Health and Welfare Fund with the decertification of the Teamsters; and

WHEREAS, authorized an alternate Health Insurance Plan for 2018 per Resolution
2018-35; and

WHEREAS, the alternate Health Insurance Plan has a higher deductible and out-of-
pocket costs for Village Employees; and

WHEREAS, the Board of Trustees would like to provide re-imbursement for deductible
and out-of-pocket costs that are greater than the previous plan with Central States Health
and Welfare;

WHEREAS, Diversified Benefit Services, Inc. has submitted a quote to provide a
Health Reimbursement Arrangement; and

WHEREAS, the Board is satisfied with the proposal.

NOW THEREFORE, the Village Board of the Village of Sturtevant, Racine County,
Wisconsin does hereby resolve:

1. That the execution of an agreement with Diversified Benefit Services, Inc.
effective May 1, 2018 is authorized and approved subject to final review and
approval by the Village Administrator; and

2. The contract will expire on December 31, 2018; and
3. That the funding shall not exceed $50,000; and
4. The Village President and the Village Clerk are authorized to sign any

agreements or other documents necessary to carry out the intent of this
resolution;

Adopted by the Village Board of the Village of Sturtevant, Racine County,
Wisconsin, this 5th day of June, 2018.
