CLIENT REQUESTS OR CHANGES IN DUTIES
It is also important that you contact your Staffing Specialist immediately if one of the following
occurs while on assignment, as your pay may be affected:
you are unable to perform the function and/or duties assigned
your job function and/or duties change dramatically or are not what you
expected
your work location changes
your supervisor changes
your work schedule changes dramatically and/or are requested to work overtime
you are requested to drive and/or travel
the client ends your assignment
PERSONNEL FILES
We maintain personnel files for all employees. These files contain documentation regarding all
aspects of the employee’s tenure with us, including performance appraisals, benefit forms, tax
forms, change of status records, and counseling notices. The personnel file belongs to us.
Active employees, however, may request to review the contents of the file by contacting their
Staffing Specialist and scheduling an in-person appointment.
To ensure that your personnel file is up-to-date at all times, employees must notify the Staffing
Specialist of any changes of name, phone number, address, marital status, number of
dependents, beneficiary designations, emergency contact information, and so forth. Changes
in these categories may affect income tax and benefit status. Documentation may be required
based on the type of change, i.e. name change requires a copy of your social security card
showing the new name, or any other appropriate legal document.
You are responsible for contacting your Staffing Specialist to discuss benefit continuation and
conversion rights in the event of a divorce, legal separation, or a dependent ceasing to be
eligible for coverage. All changes must be in writing and signed by you. Contact your Staffing
Specialist to obtain any required change form(s).
VOLUNTARY RESIGNATIONS
Temporary Solutions, Inc. will consider that you have voluntarily resigned your position with the
Company and will record it as such in your personnel file under the following circumstances:
You notify your Staffing Specialist that you no longer wish to be placed on
assignment.
