               EMPLOYMENT AND INCOME VERIFICATION

All requests for personnel information concerning applicants, current employees, or former
employees from outside sources must be directed to the Corporate Human Resource

Department. Generally, the Human Resource Department will only release information in

response to written requests after obtaining the written consent of the individual who is the

subject of the inquiry. The Human Resource Department may confirm employment dates,
positions held, and salary without written consent of the individual involved. If you would like

us to give a more detailed reference, you will have to provide us with a written release—a

consent form giving us your permission to respond to a reference request. We will respond only

to written reference requests, and we will respond only in writing. Please direct all reference
requests to the Human Resource Department.

We will comply with all subpoenas for information, documentation, etc. We will also supply
information, documentation, etc. as requested by federal, state, and local authorities without

               first obtaining your consent.

               UNEMPLOYMENT COMPENSATION INSURANCE

Unemployment compensation insurance provides benefits to eligible workers who become
unemployed through no fault of their own, and meet certain other eligibility requirements.
Monetary payments are provided to eligible workers for a given period of time or until they find
a new job. Unemployment compensation insurance is based on a dual program of federal and
state statutes so unemployment compensation may be administered differently according to

               state law.

Be aware that your failure to contact your Staffing Specialist within two (2) days following the
close of an assignment, or weekly between assignments, will be interpreted by the Company as
