How to Submit an Optum Authorization Request
Optum authorization is required for certain United Healthcare plans; auth is determined by the
plan’s group number. When Optum auth is required, the patient must complete an outcome
measure and fill out the Optum Patient Summary Form (PSF-750). These items will be used
when submitting the request, which must be completed online through the Optum Portal.
*PLEASE NOTE: Optum auth requests can only be backdated up to 10 days*
1. Log in with the Provider ID and Password obtained from Master Login In List in WebPT
External Documents.
2. In the Clinical Submissions and Claims box in the sidebar, click Submit under Clinical
Submissions. You can also select Submit a Clinical Sub from the drop down under
Clinical Subs & Claims in the light gray toolbar.
