9.  For the STarT Back Screening Tool (SBST) check No for each item and hit Calculate.
                       The SBST Category will autofill. Click Submit to submit the auth request.
























































                   10. After clicking Submit, the Patient Summary Form Confirmation Page will appear.
                       There will be a Confirmation Number (PCN) listed at the top.
                          a.  Print a copy of the Confirmation Page.
                          b.  Scan the completed PSF with the Confirmation Page and upload to the patient’s
                              WebPT eDocs. Save the document as “Optum PSF (date of completion) & PCN
                              #(confirmation #)”.
                          c.  After the forms have been uploaded in WebPT, discard in the shred box.
                   11. Update the appropriate clinic’s Authorization Log in Google Drive with the Date
                       Submitted and PCN (Response) number.
