3. Select Kiosk for the completion method and click Next.
4. Select the Outcomes Questionnaire that is most appropriate for the patient’s case.
5. Select the Type for the patient’s Initial Examination (first visit for the case), Follow Up
(re-eval), or Discharge (graduation) and then click Generate Code.
