How to Obtain a Functional Outcome Measure

               Patients must complete a functional outcome measure from WebPT on the Front Desk iPad
               prior to their initial evaluation, at any follow up appointments where the PT will be writing a
               progress note, and at their last appointment when they are being discharged. The measure is
               used as a baseline and to gauge progress throughout the patient’s treatment.

               Most commonly used types of outcomes:
                   ●  Lower Extremity Functional Scale (LEFS)
                   ●  DASH (Shoulder, Arm, Elbow, Wrist)
                   ●  Modified Oswestry (Low Back Pain)
                   ●  Neck (Neck Disability Index)
                   ●  Dizziness Handicap Inventory (Concussions)

               To add a Functional Outcome Measure to the iPad:
                   1.  In the patient’s chart, click the Patient Record Actions dropdown.
















                   2.  Select Add Outcome and press the play button.
