regarding the reason(s) for which we are seeing the patient.
a. Enter Case Title as the injury or body part being treated (i.e. R/L/B Knee)
i. Be sure to indicate if Pre-Op or Post-Op.
b. Select Primary Insurance from the dropdown.
i. Select Secondary Insurance from the dropdown, if applicable.
ii. If the patient happens to have a tertiary insurance policy (very rare), then
all standard plan information should be collected. You must email
Christine (christine.bink@webpt.com) to inform her of the patient’s tertiary
insurance so that she can add a note in RevFlow. Additionally, you must
indicate this third insurance in the Additional Info field of the case.
c. You do not need to enter any ICD codes - the PT will add these following the
eval.
d. Choose the appropriate Related Cause from the dropdown options.
e. Search and select the Referring Physician and/or PCP’s name.
i. If the patient does not have one or the Dr. does not appear, then enter
Direct Access. After saving, Add Physician and attach the Dr. to the case.
f. Select the patient’s Primary Treatment Clinic location.
g. If the patient has a Referring Physician, ask if they have any follow-up
appointments scheduled; if so, enter the Return to Dr. Date.
h. Select the Assigned Therapist as the PT who will be performing the evaluation.
i. Enter any other relevant case details in Additional Info (i.e. surgery date).
8. If the patient is going to be seen in multiple clinics or in a location other than the one that
automatically appears, then you must Add Clinic and select the appropriate Clinic Title
