Appendix 3: Five Year Strategic Plan




                     Wellness/Well-being

                            Conduct wellness environmental assessment and develop multi-year wellness program
                            Recruit wellness committee comprising leaders from various departments including
                            administration
                            Launch employee only assessment plan which consists of both health assessment and
                            biometric screening which will be weighted to assist and benchmark current employee
                            populations overall health and activity level, a foundation activity to measure future ROI

                            Launch a “Know Your Numbers” campaign and encourage employees to participate in a
                            company provided biometric test – cholesterol, blood pressure, glucose, weight, and waist
                            circumference

                            Introduce new “branded wellness” campaign to include new look, new wellness activities
                            and programs
                            Negotiate with carriers to provide a minimum of $5,000 annually to contribute to wellness
                            program costs


                     Employee Communication

                            Update and refresh in-depth and informational employee benefit guide to include more
                            detail of the benefit program in detail
                            Introduce a “Know Your Benefits” bimonthly email campaign
                            Introduce new “branded wellness” campaign to include new wellness program, new
                            wellness activities and programs

                            Communicate to ensure employees understand the connection between wellness and
                            benefit costs
