Certified Home Assessment and Modification Program

CHAMP is a contractor certification program designed to teach contractors, accessibility specialists and
other industry professionals the specifics of providing assessments and home modifications within the
workers’ compensation industry. “Providing targeted educational opportunities for improved outcomes”.

As an extension of the CHAMP certification program, the CHAMPConnect program is provided to allow
carriers and other “service seekers” to locate
and work with CHAMP certified providers
directly. A “CHAMP Direct” assignment
introduces you to a provider and the rest is
up to you. A “CHAMP Assist” assignment
provides the option to have CHAMP staff
manage all or a portion of the assignment on
your behalf.
