• Level Four: I have a special request that does not fit the other categories.

A level four assignment is ordered when the carrier or assigning party has a request
that doesn’t fit one of the other categories. A level four assignment might include

reviewing another vendor’s assessment or estimate, consulting with legal cases,
assisting with issue resolution, or providing training for their staff.

• HARP

HARP stands for Housing Assessment and Relocation Program. A HARP assignment

is given when the assigning party is needing assistance with locating accessible
housing, either temporary or permanent, new or existing, or when the parameters
of the housing issues require the carrier to consider non-traditional options.

File Set Up

Upon receiving an assignment, the DCC intake person transfers the information from the

assignment form to “GClaim”, an on-line file management system. The intake person also
reviews the assignment:

1. To determine that the assignment level is appropriate for the requested activity
2. To determine that all necessary documents are accompanying the assignment
form

3. To ensure the right DCC personnel are notified of the assignment

The assigning party should receive a confirmation of receipt of assignment within 24
hours of the assignment.

Three Phase Home Modification Process

• Phase One (Assessment / Scope Development)

The first phase is designed to establish an appropriate and accurate scope of work.
Phase one begins with the assignment of the file and it ends when an “agreed
scope” of work is established.
