Choosing a Medical Plan
You can choose from two medical plans offered by SSC administered by Anthem: the PPO Plan and the
Consumer Directed Health Plan (CDHP) with Health Savings Account (HSA). The plans provide protection for
high medical costs, focus on well-being, and include prescription drug coverage.
When you compare your options, it’s important to look at your total costs, tax advantages, and the opportunity
to invest in your future health care needs. It may be helpful to ask yourself if you want to pay more now (in
higher premiums) or later (in out-of-pocket costs).
The medical plans are a part of Anthem’s National PPO (Blue Card PPO) and offer:
• Flexibility to receive care from any doctor or hospital facility—although your out-of-pocket costs will be
considerably less if you use a hospital or doctor who participates in Anthem’s Blue Card PPO network.
• A strong provider network—you can find network providers 24/7 at Anthem.com, on Anthem’s mobile app
Sydney Health, or by calling 1-833-999-0331.
You are responsible for checking the network status of all providers and services, including specialists, labs,
etc. even if they are recommended by a network provider.
What’s the difference between the PPO and CDHP with HSA plans?
• The CDHP has a lower premium and higher deductible, and it is paired with a tax-advantaged HSA for
out-of-pocket costs.
• If you enroll in the CHDP, the company will match your HSA contributions dollar for dollar up to the
defined amount.
• The plans have different premiums, deductibles, out-of-pocket maximums, and prescription drug coverage.
