Q. Will my medical PPO or HDHP ID card information change under UMR even if I do not
change plans?
A. Yes, you will have a new member ID number, group number, as well as new phone numbers for contacting
member services and for questions about pharmacy benefits. You will also be able to download a digital
member ID card online at umr.com or via your mobile device by downloading the UMR app. On the digital
mobile ID card you will also have the option to view a QR code where your doctor’s office can scan and view
your benefits.
Q. If I go to the doctor, do I tell them my coverage is with UHC or UMR?
A. Let them know your medical insurance is through UMR, but the network they use is through UHC.
Q. How do I access my medical benefit information online with UMR?
A. By visiting umr.com on your desktop computer, tablet, or mobile device. There you will be able to Login/
Register to sign up and view your benefit information.
Q. Will I still be able to see my same doctors/providers under UMR?
A. Yes. Although the administrator will move to UMR, you will still have access to the same UnitedHealthcare
(UHC) network and providers. Because providers come in and out of network over the years, it’s always a
good idea to confirm your provider is still in‑network in the new year.
Q. Where do I go to find providers under UMR?
A. Both the PPO and HDHP plans utilize the same networks of doctors and hospitals – the UnitedHealthcare
(UHC) Select Plus network in California and the UnitedHealthcare (UHC) Choice Plus network outside of
California. To access care:
1. Go to umr.com from your device and select “Find a provider”
2. Look for the name of your provider network on your ID card –enter either UnitedHealthcare Select Plus for
CA Residents or UnitedHealthcare Choice Plus for Non‑CA Residents and start your search
3. You can also search for your provider network using the alphabetical listing or search box
4. Or call UMR toll‑free at 1.800.207.3172 and ask a representative for help.
Q. What additional resources do I have available at umr.com?
A. You will have access to view your benefits, out of pocket costs, deductibles, contact Teladoc, as well as
view your Explanation of Benefits (EOBs). You’ll also have access to the health cost estimator where you can
compare costs before accessing care, see what your out of pocket costs are prior to your health care visit
and determine what services are covered under the plan.
Q. Will my Explanation of Benefits (EOB) change?
A. Yes. Your EOB will change in format but will provide the same level of details.
Q. If I decline a medical plan now, can I enroll later?
A. No, not unless you experience a qualifying life event, such as marriage, divorce, birth of a child, or loss
of other coverage. Depending on the type of event, you may need to provide proof of the event, such as a
divorce decree or coverage termination notice.
Q. Will I need to obtain a new prior‑authorization under UMR if I’ve already received one under UHC?
A. No, you do not. UMR will be obtaining prior authorizations in place under UHC currently and honor those.
The only exception is if your prior authorization expires prior to UMR coverage start.
