        Q.  What if I forget to submit my enrollment elections during Open Enrollment?

        A.  If you take no action during the Open Enrollment window, your 2021 benefit elections will automatically roll
            over EXCEPT for any Flexible Spending Account (FSA) elections.


        Benefit Overview


        Rubrik’s 2022 U.S. benefits plan year begins January 1, 2022. The following FAQ provides an overview of the plans
        and costs for 2022.

        Q.  Who can I or my family members contact with questions?
        A.  Most information can be found on the Rubrik Benefits Microsite. If you still cannot find the answer that you
            need please contact the internal Rubrik Benefits Team via email benefits@rubrik.com or the Slack channel

        Medical Plans



        Q.  What is the difference between the UMR PPO and UMR HDHP plan?
        A.   All plans provide access to the same network of providers and allow flexibility to self‑refer to a specialist
            without requiring a referral from a primary care physician. Watch this video to compare the PPO and HDHP
            plans. Following is a brief description of the plans:
                Ÿ  UMR PPO: Members have flexibility to access care in or out of network, and members share in the cost of
               care through copays and coinsurance. Many services are not subject to the deductible, including office visits
               and prescriptions.
                Ÿ  UMR HDHP: As indicated by the name, this plan includes a high deductible. All services with the exception
               of preventive care are subject to the deductible. This means you will pay for the full cost of care including
               doctor visits when you are sick or picking up prescriptions. After the deductible is met, members share in
               the cost of care through copays and coinsurance. The deductible and post‑deductible coinsurance costs
               that you are responsible for will be capped at the out‑of‑pocket maximum. The benefit of the HDHP is that it
               allows you to contribute to a Health Savings Account (HSA). If enrolled in the Rubrik UMR HDHP, Rubrik will
               fund your HSA with HSA Bank every pay period. See plan details for more information.

        Q.  When will the plan year deductibles (UMR medical and MetLife dental plans) reset?
        A.  Plan benefits, which includes deductibles, out‑of‑pocket maximum accumulations, and/or annual benefit
            maximums reset every calendar year (January 1 ).

        Q.  If I enroll in a medical plan, when can I expect my ID cards from UMR or Kaiser, and should I
            expect them for my spouse/domestic partner/children?
        A.  All UMR PPO and HDHP members and newly enrolled Kaiser members will receive their ID cards on or
            around January 1, 2022. UMR provides one medical ID card to members with single coverage and two cards
            to members who have two or more covered dependents. The ID cards will list the employee’s name and
            member ID number as well as the name of each covered dependent. If you are newly enrolled in Rubrik’s
            Kaiser plan but you’ve enrolled in Kaiser previously (whether with Rubrik or another employer), you may not
            receive a new card and may use your old card as your Kaiser Member ID will be carried over.
