        Q.  Does my deductible apply to my annual out‑of‑pocket expense?

        A.  Yes. All covered expenses that are paid out‑of‑pocket, including the deductible, will apply to your out of
            pocket maximum. Once you have met your annual out‑of‑pocket maximum, the plan will pay 100% of all
            covered expenses for the remainder of the plan year. Please note when accessing care out‑of‑network only
            the allowed portion of the medical expense will apply to the out‑of‑pocket maximum.
        Q.  How can I estimate my expenses for office visits and prescription medications?

        A.  The best way to determine the cost of your health care is to review your Explanation of Benefits (EOB).
            Each time you visit your doctor, access care through a lab facility, fill a prescription, or any other medically
            necessary service, your provider will bill your insurance. After your insurance carrier (UMR) has reviewed
            the claim, they will issue an EOB outlining the billed charges, negotiated amounts, and your payment
            responsibility. Typically, when you are accessing care under a high deductible health plan, you will be
            responsible for paying the full negotiated amounts until you have satisfied your deductible. UMR members
            may access their EOBs online. Also, UHC offers a cost transparency tool that can assist with finding your

        Health Savings Account (HSA)



        Q.  What is a Health Savings Account (HSA)?
        A.  A health savings account (HSA) works like an IRA. You deposit money tax‑free* and it grows tax‑free until you
            use it. A health savings account is an individually owned account that allows you to set aside pre‑tax* dollars
            for qualifying medical, dental and vision expenses. Interest or dividends accumulate tax‑free, and payments
            for HSA qualified medical expenses have no additional tax consequences. Plus, you decide how to invest and
            grow your HSA.
            *CA and NJ do not recognize HSA as a tax‑advantaged account.
        Q.   What bank does Rubrik use for the HSA account, and how do I enroll in the HSA?

        A.   Rubrik’s HSA is administered by HSA Bank. Enrollment in the HSA is separate from the UMR HDHP plan.
            Please do not waive the HSA benefit in Workday if you elect the HDHP plan. If you do not wish to submit
            personal contributions, simply elect a $0 contribution in Workday. Waiving the HSA benefit in Workday will
            opt you out of receiving employer contributions. If you are newly enrolling in the HDHP with HSA, Rubrik will
            open your HSA Bank account on your behalf. Be on the lookout for mail from HSA Bank with your account
            details and your HSA Bank debit card which will arrive 2‑3 weeks after your account has been established.
        Q.  When can I make contributions to my HSA?

        A.  You are eligible to establish and contribute into your HSA on the first day of the month coincident with
            or following your enrollment in the HDHP. After your account is established, contributions from both you
            and Rubrik may be made to your HSA. Before your account can be established, the Patriot Act requires all
            financial institutions to obtain, verify and record information that identifies each person who opens an HSA.
            This means that when Rubrik submits enrollment to HSA Bank on your behalf, HSA Bank will obtain your
            personal information (i.e. your name, street address, date of birth and other information to establish and
            confirm your identity). This process takes approximately two days, during which time your account will be
            blocked. Once this process is completed and your identity has been verified, access to your HSA will be
            unblocked and made available to you. If your identity is not verified (e.g. if you moved recently and your new
            address is not on file with the appropriate government agency), you may be asked to provide proof of your
            identity by providing a copy of your utility bill to verify your address or a copy of your Social Security card if
            the number does not match the verifying source’s records. Please respond promptly to any correspondence
            from HSA Bank.
