Employees electing domestic partner benefits would be well advised to consult an attorney
regarding the legal and tax implications of accepting insurance coverage and other benefits
from the NWF Employee Benefits Program applicable to a domestic partner relationship and
signing the Affidavit of Domestic Partner Relationship.

Employees will be eligible for benefits under the Sick Leave, Family Medical Leave, Leave of
Absence, Bereavement Leave, and Employee Assistance Program policies for situations
covered by these policies relating to their domestic partner relationship provided the
employee has previously documented the domestic partner relationship by executing the
NWF Affidavit of Domestic Partner Relationship, and filing the Affidavit with the Sr. Director
of Total Rewards in the Human Resources Department.

Any absence from work requesting paid leave or leave-without-pay through the Wellbeing
Leave, Sick Leave (hired prior to 2020), Family Medical Leave, Leave of Absence, or
Bereavement Leave policy, relating to an NWF recognized domestic partner, is to be
       recorded on the employee's timesheet.
