No clear tagging and numbering on visual board for community list visit

-Numbering was tagged to the visual board and folder patient to help staf to trace folder patient
-List name of patients of assertive communty visit, acute home care, proxy visit, defaulter tracing and social visit list of patient was clearly
separated and tagged on the visual board. date of visit also stated
