Employees submitting treatment costs to the Company benefits plan may not also receive employer
          half (½) cost coverage of treatment. Treatments submitted to any benefits plan will be included in the
          maximum of twenty-two (22) session allotments because the treatment is received on company time.
          JUST TO BE CLEAR: If you receive in-house massage treatments and Purity Life pays for half (½) the cost
          (example $31), you and/or a family member cannot claim the portion paid through payroll deduction
          to your Group Benefits Plan under massage for reimbursement. Purity Life will be instructing our on-site
          professionals not to give receipts unless you are not taking the Purity Life discount. These costs affect our claims
          experience on our plan and, therefore, increase our costs.
          Sessions do vary in length, so for the purpose of ensuring equality, one (1) session is equivalent to forty-five (45)
          minutes or more. All head-office treatments are booked through reception at extension 221. Please note: Some
          practitioners have limited time and book up quickly. Therefore, to ensure fairness among employees, you may
          only be able to book with certain practitioners on a monthly basis.

          Supplement Wellness Program
          Purity Life is excited to invest in your health. All Purity Life employees will receive a three month supply of
          Moducare, Quest Vitamin D, Kyolic Extra Strength One A Day and Quest Daily One Multivitamin. We believe
          based on the health professionals we work with, as well as research around these products and ingredients,
          that they are part of a foundation for good health. If you are interested, please see the HR Department to obtain
          literature and obtain your free supplements.

          Healthy Snacks Program
          On January 26, 2016 Purity Life announced its new Healthy snacks Program. It is Purity Life’s goal to make our
          workplace a healthy and great place to work. As part of this continuous improvement effort we are expanding
          our Wellness Program to include healthy snacks options. Purity Life has purchased a vending machine and has
          stocked it with healthy snacks that we currently distribute. These snacks will be available to you at the amazing
          low price of $0.50 to $1.00, Purity Life will be subsidizing the rest to invest in your health. Checkout all the great
          options in the cafeteria today!

          Staff Purchases
          A benefit of working at Purity Life is the staff discount plan. All current employees are eligible to purchase
          products at wholesale cost less 20% (some exceptions apply) for their own or immediate family use. All staff
          purchases will be deducted from your next pay cheque or paid via personal credit card. No other payment
          options will be accepted. Please note: See Policy for complete terms and conditions as exceptions do apply.
           	 Purity Life provides employees with online ordering. Please see Director, Customer Service to get
          	 enrolled in this program. The web address to order products is http://customerlink.puritylife.com.

          Education Reimbursement
          Purity Life will reimburse full-time employees the cost of continuing education (academic courses, seminars, or
          conferences, where the primary objective is education). Management pre-approval of the course is required. We
          do our best to make the education privilege available to everyone; however, there are times when funding may
          be limited.
