g.  Is the patient’s family at bedside?

                       3.  The request may be passed on by staff but should originate with the
                          patient or the family.

                       4.  If the request is for a Priest the Spiritual Care Professional, Resident or
                          patient care unit, will page the Roman Catholic Archdiocese for a Priest.

                       5.  The Spiritual Care Professional, Resident or patient care unit provides the
                          above information to the Roman Catholic Archdiocese. If the priest is in
                          need of any further information, the individual who called the Roman
                          Catholic Archdiocese will be called for follow up.

                       6.  In the event that a Priest does not come in, the Spiritual Care Professional
                          or Resident will follow up with the Department of Spiritual Care
                          manager(s) who will in turn follow up with the Roman Catholic
                          Archdiocese of Toronto.


                                                    PROCESS STARTING POINT
                         Requests for a Priest are made through the on-call Spiritual Care pager or directly
                                   to the Roman Catholic Archdiocese from the patient care unit.
                                                             
                         The Spiritual Care Professional or Resident,  if on duty, responds to all requests
                         providing necessary Spiritual Care and determines the following information:
                                   a.  Request is initiated by the patient or family
                                   b.  Verify patient is Roman Catholic
                                   c.  Name and location of patient
                                   d.  Verify reason for request: Sacrament of the Sick, Communion or
                                       Confession
                                   e.  Is the patient in danger of immediate death?
                                   f.  Is the patient conscious?
                                   g.  Is the patient’s family at bedside?

                                                             
                         The request may be passed on by staff but should originate with the patient or
                         the family.
                                                             
                         If the request is for Sacrament of the Sick, communion or confession the Spiritual
                         Care Professional, Resident or patient care unit will page the Roman Catholic
                         Diocese.
