University, nor has a criminal case before any court of justice at the time
of selection.
7.3. In good physical and mental health as certified by a government
physician.
7.4. Has permanent status and has served the University for at least one year
and at least with “very satisfactory” performance during the last two
rating periods immediately preceding the date of application.
7.5. Upon approval of the scholarship grant, grantee must secure admission
notice from the University/College where she/he intends to pursue the
degree.
8. PROCEDURE:
Complete application documents for any of the staff development
opportunities particularly on scholarships, and training grants with more than
one (1) month duration (competitive) must be submitted one month or more
before the commencement of the requested grant/leave as the case may be.
The process of application is as follows:
8.1. Applicant secures FSD Application Form from the FSDC
Secretary/DHRD and submits the same together with the required
attachments to his/her immediate supervisor for evaluation and/or
endorsement.
8.2. The immediate supervisor (Program Chairperson/Section Head)
forwards the applicant’s application to the next higher supervisor
(Dean/Department Head/Campus Administrator) stating his/her
justifications why he/she is endorsing the application or not
8.3. If the College/Department Head/ Campus Administrator’s action is
positive, the same is forwarded to the FSD Committee Secretary for
evaluation as to compliance to requirements then notifies the FSDC
for an en banc deliberation on all applications.
8.4. The FSDC meets for the deliberation on all applications, summarizes
. the results ■ and submits to the University President its
recommendation/endorsement for each and every applicant An
interview of the applicants may be undertaken, if necessary, to
ascertain and confirm the applicants’ commitment to pursue and
complete the grant. If the FSDC cannot meet, a
recommendation/action through a referendum can be routed for them
to act on the application to be summarized as a committee output
by the FSDC Secretaiy.
8.5. The Committee’s action must always be supported by a unanimous
decision in support to an application before it can be endorsed for the
President’s action. Committee comments and objectives must be fully
addressed by the applicant.
8.6. The University President acts on the recommendations and returns the
same to the FSDC for implementation.
8.7. The grantee enters into a contract with the University.
