The purpose of this list is to provide the Manager or Team Member with a series of questions to be aware of in order to
            help the authorities quickly identify the suspect and reduce or minimize the threat. Be Calm, Be Courteous and Listen; Do
            Not Interrupt the Caller. Write down as much as you can, capturing what the caller said:
