In case of special mobile device model requirement, the request will also be approved by CH (L1) level or
higher, CFO, CPO and CIO. The approved request will then be forwarded to Human Resources
Department to proceed.
Procedures and Criteria
1. General Terms
1.1. A company mobile devices (phone and tablet) as well as its standard auxiliary peripherals are a part
of company asset lent to the employee for the daily work. Employee must safeguard them and as if it
is employees’ own property and return them to the company by his/her resignation.
1.2. One employee is usually allocated or can possess only one company phone and one tablet device at
a time and for a certain period of time prior to the replacement.
2. Model Eligibility and Exceptional Requirements
The company phone and tablet model will be made eligible to employees based on their job level as well
as the IT Policy and Guideline.
Any exceptional requisition or extra requirement of mobile phone and tablet on top of the company
standard equipment, the Employee must obtain the Business Manager of CH (L1) level or higher, CFO,
and the request must be approved by CPO.
3. Requisition Process
All company mobile phone and tablet device will be arranged by the Administrative Department following
the standard company asset management procedures. (See Asset Requisition Form)
4. Equipment Accountability
Employee will take full responsibility for the mobile phone and tablet, ensuring that all necessary
precautions are taken to safeguard its security and condition. The mobile devices must be used solely by
the employee, unless written permission is been given for another individual or group to use it.
5. Negligence
If device is lost /stolen/damaged and it is proven that the employee fails to take the necessary precautions;
the employee must pay for all replacement/repair costs.
In case of lost or stolen, employee must make immediate notification to police officer, Human Resources
and IT Administrator. A copy of the police report must be acquired within one day after the incident and
submitted to Human Resources and IT Administrator together with the new iPad replacement request.
