Signs of pressure sore
Redness & inflammations
Blister the whole dermis
Breakage of the skin (ulcer) & subcutaneous
Necrosis (death of tissue)
Prevention
Change the patient's position frequently
Keep bed free from wrinkles and crumbs
Keep bed dry
Use air rings, cotton rings, and an air mattress to relieve pressure
Keep clean patient clean
Give back rub frequently, giving special attention to bony prominences and reddened
areas.
4.4 Back care
Purpose
To stimulate circulations
To relax patient
To prevent pressure sores or decubitus
Equipment
1. Soap and water
2. Bath towel (bath blanket)
3. Wash cloths
4. Powder
5. Other medications as ordered, castor oil, zinc oxide, tincture of benzoic, boric acid, etc.
6. Air ring, cotton ring as necessary
7. Pillow
Procedure
1. Place the patient on his side (prone in contraindicated)
2. Place the towel under the patient's side
3. Wash with soap and water dry
4. Apply powder or lotions are not used together and rub back. Pay special attention to
prom incenses.
5. Use firm, long strokes upwards and downwards in a circular motion.
6. Circular movement should be used on bony prominences.
7. Repeat upward and downward stroke several times.
8. Dry back and apply powder
9. Turn the patient on the back and put the pillow as necessary
