                       student organizations.


                                         Accreditation of Student Clubs/Organizations



                                                  Submit Financial Report

                            The last two items are required of student clubs and organizations
                                                 requesting for re-accreditation






                                     Submit Accomplishment Report to the DSSD Office






                         Furnish Letter of acceptance of the faculty adviser chosen by officers
                         and members of the club/organization in the DSSD Office








                               Submit Constitution and By-laws of the club/organization;







                          Proposed Calendar of Activities and Action Plan of the organization for
                                                        the School Year;
