1. Present caregiver for minor care.
2. Marilyn Maxwell, Grades PS - 5 for basic care/ill child can administer emergency medications if
medicine has been provided to her. If she is not available, parents are to be called.
3. Study Hall Proctor, Grades 6 – 8, for minor care.
4. In an emergency, if Patty DePasquale is not on campus, call 911.

Additional Information
1. Most Child Care Personnel have been CPR/First Aid Trained.
2. Most Coaches have been CPR/First Aid Trained.

8:00 a.m. – 1:00 p.m. Fri.
4. Automatic Defibrillator and Generic Epipen are behind the Front Desk.
Generic Epipen can only be administered to an undiagnosed patient by Patty DePasquale
or Lisa Terwilliger during school hours.
5. Grades 5 - 8 students can carry their inhaler as long as a doctor’s note has been provided to the
nurse’s office.
6. Grade 6 – 8 students participating in athletics must provide coach with separate medications including
epipen and doctor’s authorization.
7. Emergency Forms: Nurse’s office/Front Desk: All students:
Peter Reynolds/Coaches – Grades 6 – 8
Marilyn Maxwell – Grades PS – 5
Teachers – All students in class
8. Faculty/Staff Medical Forms are in the Nurse’s Office.
9. Under no circumstances should the nurse’s office be accessed after school hours. Call 911.
NON-CUSTODIAL PARENT INFORMATION
In the case of divorced or separated parents, the school makes every effort to be fair to both parties, without
getting involved in disputes. For legal reasons, the school may not refuse access to either parent without a
court order so stating. In addition, all written materials will be sent to both parents unless a court order directs
differently. Both parents must sign all contracts before they are valid.
