are safe, properly documented, conveyed to the responsible management authority
and approved. The Engineering Change Authority procedure controls any changes
in:
• Process and Instrumentation flow;
• Materials of construction;
• Equipment sizing, type;
• Plant layout; and
• Environmental control facilities.
C.2.7 Incident Reporting and Investigation
Incident reporting, investigation, and sharing of lessons learned
Accident Investigation Procedure (SAJ-HSE-P-40) established to act as a guideline
to perform incident investigation. An investigation team will be formed which will
include a member from the EHS Department to carry out investigations. The
investigation report, shall be sent to all the managers, the Chairman of the Safety
and Health Committee and the Safety and Health Committee members. Depending
on the seriousness of the accident, the details of the accident and the results of the
investigation are presented to the Chief Executive Officer of RSAJ.
The incident reporting system includes the following:
• Date and location of the incident;
• Description of the incident;
• Factors contributing to the incident;
• Immediate actions put in place;
• Apparent severity level of the incident;
• Date of the investigation;
• Composition of the investigation team;
• Follow up actions resulting from the investigation;
• Responsibilities for follow up actions;
• Completion dates; and
• Approvals
Whenever any accident occurs at the workplace which causes any serious bodily
injuries or death then the employer shall notify the nearest Department of
Occupational Safety and Health office (DOSH) by the quickest means and also
within 7 days send a report thereof in an approved form.
