                   •  Accident records ‐ internal and/or industry-wide;
                   •  How much harm can be reasonably foreseen;
                   •  How many people might foreseeably be affected;
                   •  Is the incident likely to happen without warning or will there be time to escape
                       it; and
                   •  Are the effects short or long term.

               Estimations of likelihood by taking account of:
                   •  Incident/accident records - internal and/or industry-wide;

                   •  Near miss experiences;
                   •  How often the activity is undertaken (e.g. daily, weekly, monthly, etc.)

               There are also several types of risk assessment tools implemented and the selection
               will depend on the type and complexity of the activity under consideration as follows:
                   •  Job Hazard Analysis (JHA);
                   •  Hazard Identification (HAZID) workshops; or
                   •  Hazard and Operability (HAZOP) studies.

               C.2.8.3 Recording of Risk and Risk Register

               The Company maintains a risk register based on the processes. For non-routine
               activities and tasks, a risk assessment will be conducted to identify the hazard, risk,
               and suitable controls.

               C.2.9 Safety Review / Studies for Critical Tasks

               Arrangement for safety reviews or studies undertaken before the execution of
               critical tasks

               As part of the Company OSH management process, any critical tasks or activities
               such as pre-start-up  of the  new system or system  which has been repaired  or
               service and ready for reinstatement will subject to proper risk assessment and permit
               to work process.

               The personnel participating in the operations should be trained and inducted to carry
               out the essential tasks or operations. The supervisor who performs the role of the job
               leader shall ensure that all hazards and risks are identified, assessed and necessary
               steps  are taken before pursuing the task. If  any  formal safety studies  or review
               required, it will be implemented and provided for internal review by the work team.

               The level of safety studies and review will in line with the local legislation.
