• Information requirements that include equipment identification, description of
the inspection or test, the condition of the equipment and the results;
• Records of inspection and testing results, repairs or alterations;
• Technical knowledge required; and
• Mandatory testing of pressure vessels and equipment.
The inspection and testing program is regularly reviewed for appropriateness, design
limits, accuracy and have a valid calibration certificate. The system includes
independent inspection testing where necessary to verify the adequacy of the
inspection and testing program, the integrity of processes, plant, and equipment.
All pressure testing is carried out under strict Permit to Work conditions and
performed by the approved and competent personnel to carry out the task. Test
areas are isolated and all valves and instruments are identified and isolated using
the isolation procedure.
C.2.4.2 Risk-Based Inspection
A risk-based inspection (RBI) strategy is used for piping systems and pressure
containing equipment items. The RBI concept is based on a thorough understanding
of the processes and operating conditions together with the maintenance and
inspection history of the equipment and piping systems. The inspection programs
and schedules are defined based on detailed studies related to the operational
aspects of the equipment and piping systems.
The Company has identified inspection frequency based on the associated risk of
failure and criticality of the said equipment when on demand.
C.2.4.3 Pressure Relieving Valves Test
In line with codes and standards, all pressure relief valves are subject to inspection,
overhaul and calibration. All tests are witnessed and approved by the Inspector
before the valve is allowed to be re-installed at the plant.
C.2.4.4 Maintenance
The maintenance regime of the plant has the following objectives:
• To preserve the investment in the existing facilities and meet their functional
requirements; and
• To maximize integrity, reliability, availability, and utilization of plant and
equipment.
