location and fill out an “OUT OF SERVICE” tag. Place this lockout
tag on the start button.
4-203 LOTO on systems only after receiving arc flash training. When
operating disconnects (LOTO), wear the appropriate arc flash PPE
according to the applicable arc flash label. ALWAYS stand to the
side of the disconnect (NEVER stand directly in front). Use your
hand to pull the disconnect switch to the “OFF” position, look away
and hold your breath. NEVER stand directly in front of the
disconnect switch when operating disconnect switches to either
the “OFF” or “ON” position.
4-204 With the disconnect switch in the “OFF” position, attach your
personal safety LOCK and TAG and place the key(s) in your
pocket. Do not give your key(s) to anyone. If there is potential for
more than one person attaching a lock/tag, ALWAYS place the
hasp on the disconnect first. Only five locks are permitted on one
hasp. If more than five employees are working on the equipment,
a second hasp is required.
4-205 Before working on any equipment that you have locked out and
tagged out (LOTO), TEST the equipment for safety. Try to start the
equipment at the particular stop/start station or control panel. If
more than one person is required to LOTO, a designated and
trained employee is required to test the equipment. All others are
to confirm the testing procedure was followed. After testing,
ensure the control switch is placed in the off position.
IMPORTANT: Testing the start equipment from computer
control system shall be done by a designated and trained
employee.
4-206 LOTO equipment that feeds to and discharges from the equipment
that is under repair. Obtain additional locks and tags at your LOTO
station or from your supervisor if needed.
4-207 When work on a piece of equipment is not complete, the
supervisor must be informed before the last person removes his or
her lock. The supervisor must then replace the lock with his or her
own LOTO.
4-208 No employee may remove another employee’s lock and tag.
Only a supervisor may remove an employee’s lock and tag.
Vulcan allows removal only after repeated and documented
attempts to locate the employee. In such cases, a supervisor must
complete a “Master Key LOTO Removal Record.”
4-209 Call a qualified electrician to disconnect equipment that cannot be
locked out. E-stops/pull cords shall not be used as a means of
disconnect.
4-210 On major construction projects (not repair and maintenance) the
following must have LOTO on the appropriate disconnects for the
duration of the project:
