4-211 Violations of the lockout and tagout (LOTO) procedures will not be
tolerated. Employees who violate these requirements will be subject
to disciplinary action up to and including termination. Termination
may be administered on the first offense
LOCKOUT, TAGOUT AND BLOCKOUT PROCEDURES
– MOBILE EQUIPMENT (4-300)
4-300 Whenever a piece of equipment or machinery requires detailed
inspection, cleaning, repair, or service, it must be electrically and
mechanically de-energized before any employee may begin work on
the equipment. Each employee will be required to use a safety lock
and tag to successfully complete the de-energizing task.
4-301 The following procedures (Rules 4-302 through 4-308) will help to
ensure the equipment is in a safe condition to proceed with repairs.
These are general rules to follow and may vary with each piece of
equipment. For specific instructions refer to the manufacturer’s
service manual.
4-302 Park equipment in a safe location on firm level ground, with
transmission in park and the parking brake set.
4-303 Lower all implements and attachments to the ground or support with
OEM or OEM engineering-approved stands, proper cribbing or OEM
supplied pins.
4-304 Connect the steering frame lock between the front frame and the
rear frame on articulated equipment before working in areas where
articulated movement can cause injury. Refer to the OEM manual. If
a machine has to be articulated to perform the work, follow the OEM
instructions for bleeding down the accumulators and creating a safe
condition to prevent movement.
4-305 Remove ignition key, place battery master disconnect switch to
the “OFF” position, if equipped (at the end of every shift and when
the machine is being worked on). If capable, place LOTO on master
switch and secure the key. Ignition key and lock key shall be kept in
your pocket.
4-306 If a disconnect is not provided and if the work involves welding,
major component removal or electrical repair, remove the ground
cable from the battery to prevent accidental startup, damage, etc.
4-307 Chock wheels with the appropriate size chocks.
4-308 Attach “Do Not Operate” tag to equipment in a highly visible
location. (i.e., steering wheel, entrance steps, battery master
disconnect switch, transmission lock lever, ground shut-off switch,
etc.)
