3-212   Because of electrical hazards, never use aluminum ladders.

                               3-213   Do not use the extension part of an extension ladder as a base
                                       ladder. Always be sure that extension ladders extend above the
                                       upper landing surface by at least three rungs.

                               3-214   Face the ladder when ascending or descending and use both
                                       hands for stability. Always face the ladder when working.

                               3-215   Do not place a ladder on objects to obtain additional height.

                               3-216   If using a ladder as a work platform and 3 points of contact
                                       cannot be maintained, fall protection must be used if working at
                                       heights greater than six feet or if working over a hazardous
                                       condition (e.g. impalement hazard).

                               Fall Protection

                               3-217   Receive adequate task training on the proper use and inspection
                                       of fall protection equipment prior to initial use.

                               3-218   Mandatory use of fall protection is required if working outside of
                                       handrails at an elevation of 6 feet or greater. If working in an
                                       area outside of handrails under 6 feet, fall protection may still be
                                       required when you are working above a hazardous area.

                               3-219   Fall protection equipment should be inspected prior to each use.
                                       If defects are found it must be taken out of service.

                               3-220   Any fall protection equipment involved in a fall must be taken out
                                       of service.

                               3-221   Lanyards must be attached to adequate anchorage points,
                                       capable of supporting at least 5,000 lbs. Unless specifically
                                       designed, a lanyard must not be attached back to itself. Use of
                                       beam clamps, beam straps, etc, should be considered when
                                       planning fall protection.

                               Additional Precautions

                               3-222   When lifting, hold your back straight, bend your knees, get a firm
                                       grip on the object, hold it close to your body, and space your feet
                                       for good balancing. When you lift, exert the lifting force with your
                                       strong leg muscles, not the weaker back muscles. Never lift
                                       anything that is too heavy or awkward. Always get help.

                               3-223   Wear a US Coast Guard (Type I, II, III) approved life vest at all
                                       times when working on water or near water (within 7 feet) or
                                       where there is a danger of falling into water. If inside of handrails
                                       or other fall prevention, a life vest is not required.
