b) Execution Meetings
HSSE matters shall be the first subject on the agenda in all site meetings held between the
Employer and the Contractor.
4.9. Risk Assessment
Contractor shall develop and implement all risk management actions which are applicable for site as
may be identified in Hazard Identification, Risk Assessment and Risk Control (HIRARC), Environmental
Impact Assessment (EIA), or other risk assessment studies.
a) Hazard Identification, Risk Assessment and Risk Control (HIRARC)
When planning method of work, a suitable and sufficient assessment should be carried out and
recorded. Method, materials, and equipment should be selected to remove or minimize risk from
Works. Contractors are responsible to carry out HIRARC for all activities at worksite in accordance
with DOSH Guideline for Hazard Identification, Risk Assessment and Risk Control, 2008.
The principles of risk assessment listed below should be adhered to when determining methods
and sequences of Works:
I. Identification of the hazards involved for each step of the proposed Works;
II. Assessment of the risk (likelihood and severity) of any potential harm arising;
III. Removal of risks, possibly by changing the proposed methods or processes;
IV. Control of remaining risks according to hierarchy of controls (Eliminate, Substitute, Isolate,
Engineering, Administrative and PPE);
V. For high and medium risks, the contractor shall prepare and implement further action
control to reduce the risks;
VI. Review, and if appropriate, update.
b) Job Safety Analysis (JSA)
Contractor is required to develop complete Job Safety Analysis (JSA) prior to commencement of
Works at site, and to be reviewed by Allo. JSA focuses on job-specific risks and are typically
performed for every single task by assessing each step of the Activities. In JSA, potential hazards
of each steps of the Activities are identified and the safest way to do the Activities will be
recommended by relevant parties.
c) Four basic stages to be included in the preparation of JSA:
I. Selecting the Activities to be analysed
II. Breaking down the Activities into a sequence of steps
III. Identifying the potential hazards
IV. Determining the preventive measures to overcome these hazards
d) Chemical Health Risk Assessment (CHRA)
The contractor shall carry out a written assessment of the risks arising from the use of chemicals
that is hazardous to health by the workers as required by Part IV of Occupational safety and
Health (Use and Standards of Exposure of Chemicals Hazardous to Health) Regulations 2000.
CHRA shall be by Competent Assessor, approved by DOSH.
